Medical Records

Our patients can have access to their own medical records by signing a release of information form. Patients often request their records in order to receive continued care elsewhere (especially if they are seasonal residents) or just to keep them for their own personal records. The physicians involved with their care also have access to patients' records in order to review the patient's previous treatment. Advance directives, such as a living will, are also kept in your record.

If you want to obtain a copy of your medical records, please follow directions below:

  • Complete, sign and date the Authorization for Use or Disclosure of Protected Health Information form. You may download and print the form by clicking: HIPAA Authorization for Release of Information. Provide a legible copy of the patient/legal representative’s Photo ID with signature. If legal representative, please provide supporting documentation (i.e. Death Certificate, Power of Attorney for medical records, Healthcare Surrogate).

    Fees

    Once your request has been processed, Charlotte Regional will send out the copies of your record along with an invoice. Charges may include $1.00 per page plus retrieval fee, tax, and postage. Payment shall be remitted via check or credit card to Charlotte Regional Medical Center. The fees are waived if we provide the copies directly to your doctor for continuation of medical care.

    Request Turn Around Time

    Most requests are completed within seven to ten business days. However, delays may occur depending upon the availability of the records and the completeness of the request.

    Faxing of Records

    Copies of your records cannot be faxed directly to you. We fax copies of your medical records only to other healthcare providers in emergency cases.
